MME was engaged to perform failure analysis pertaining to a train derailment that was initiated by a fatigue fracture of a single car axle after preliminary investigation by several parties failed to reveal a singular root cause.

MME undertook a comprehensive investigation involving metallurgical, mechanical and structural engineers as well non-destructive testing specialists. The results of MME’s investigation identified a number of factors, that, when acting in conjunction, were responsible for the derailment.
